GapCoverage™ Terms & Conditions

1. Introduction:

Welcome to the GTR Sante Blood Testing Program. By using our services, you agree to comply with and be bound by the following terms and conditions. Please read these terms carefully before accessing or using our website.

2. Eligibility:

a. To participate in the GTR Sante Blood Testing Program, individuals must have private insurance or private group insurance.

b. Deductibles are not covered by GapCoverage™.

c. GapCoverage™ is not applicable for semi-private or reduced pricing blood testing.

d. GapCoverage™ is not applicable for exempt tests or tests not covered by private insurance

e. GapCoverage™ is not applicable when insurance limits are reached or exceeded.

f. GapCoverage™ is not applicable on Mobile Service fees.

f. In Quebec, private labs are not approved for advanced reimbursement, requiring clients to pay in advance and submit claims to their individual insurance.

3. Coverage Tiers: a. Coverage percentages are as follows:

80% Coverage: Lowest retail price is $150.
70% Coverage: Lowest retail price is $175.
60% Coverage: Lowest retail price is $200.
50% Coverage: Lowest retail price is $300.
100% Coverage: Free gift card.
Less than $400: $50 gift card.
Over $400: $100 gift card.
Gift cards cannot be used for mobile fees and are only available for GTR Sante services.

 

 

 

4. Quotation and Payment Procedure:

a. Clients are quoted for private blood testing based on their testing requisition.

b. Clients pay in advance for the quoted services.

c. The first receipt from our booking software serves as proof of payment.
d. Within 24 hours from service, clients receive a detailed invoice to submit to their insurance for reimbursement.

5. Insurance Reimbursement:
a. After receiving reimbursement from their insurance company, clients must fill out the GapCoverage™ Insurance Refund Form with proof of reimbursement at https://www.gtrsante.com/pages/gapcoverage-refund-form
b. More details may be requested to approve the refund.
c. Approval can take up to 72 hours from submission of form.
c. Once approved, refunds will be processed directly through the original payment method. It may take up to 5 business days to appear in your account.

6.  Refund Policy:
a. Refunds are not guaranteed.
b. Clients can send their policy to help@gtrsante.com for assistance in reviewing coverage details if they are unsure.
c. Please refer to Section 2. Eligibility.

7.  Discretion and Guidelines:
a. The use of GapCoverage™ is subject to individual client needs and insurance coverage.
b. GTR Sante reserves the right to exercise discretion in offering GapCoverage™ based on varying circumstances.

8. Modifications: GTR Sante reserves the right to modify or amend these terms and conditions at any time. Users are responsible for regularly reviewing these terms to stay informed of any changes.

9. Acceptance of Terms:
By using our GapCoverage™ services, you signify your acceptance of these terms and conditions.
